 The employee shall abide by this job description, his/her employment contract, and the PU-AMI Afghanistan National Staff Regulations, including their annexes and attachments, as well as any rules, regulations, and internal policies that may be adopted by PU-AMI Afghanistan.

Overall Objective

Under the supervision of the Programme Manager, the Medical Doctor is responsible for providing comprehensive, high-quality, and patient-care - as per Ministry of Public Health (MoPH) Afghanistan, BPHS/EPHS guidelines and organizational medical protocols.

The Medical Doctor provides curative, preventive, promotive, emergency, maternal, newborn, child, reproductive health, and referral services while ensuring medical ethics, patient safety, confidentiality, and quality of care. The position contributes to improving the health status of vulnerable populations through evidence-based clinical practice, health promotion, disease prevention, and collaboration with multidisciplinary health teams.

Key Responsibilities

·        Primary General Clinical Service Delivery

• Provide comprehensive outpatient consultations for women, children, and adult patients according to BPHS standards.
• Conduct thorough history taking, physical examinations, diagnosis, treatment, and follow-up of patients using national clinical guidelines.
• Prescribe medicines rationally according to the Afghanistan Essential Medicines List and Standard Treatment Guidelines.
• Perform minor medical procedures within the approved scope of practice.
• Identify and manage medical emergencies and stabilize patients prior to referral when necessary.
• Provide follow-up consultations for patients with chronic illnesses and monitor treatment outcomes.
• Ensure all patients receive respectful, confidential, and non-discriminatory care.

2. 3. Sexual and Reproductive Health and maternal and newborn child Health

• Provide counselling on family planning methods in accordance with national guidelines.
• Diagnose and manage reproductive tract infections and sexually transmitted infections.
• Participate in child and nutrition screening.
• Promote reproductive health education for women and adolescents. Collaborate with midwives to improve maternal and newborn outcomes.
• Ensure privacy, dignity, and confidentiality during all reproductive health consultations.

4. Management of Communicable Diseases

• Diagnose and manage communicable diseases according to MoPH protocols.
• Identify suspected cases of tuberculosis, malaria, acute watery diarrhea, measles, leishmaniasis, COVID-19, and other priority diseases.
• Immediately notify suspected outbreaks through the established disease surveillance system.
• Participate in outbreak preparedness and response activities.
• Ensure appropriate infection prevention and control practices during patient care.

5. Non-Communicable Diseases (NCDs)

• Screen, diagnose, and manage patients with hypertension, diabetes mellitus, asthma, chronic respiratory diseases, epilepsy, and other common NCDs.
• Provide counselling on healthy lifestyles, nutrition, physical activity, smoking cessation, and medication adherence.
• Ensure regular follow-up and continuity of care for chronic disease patients.

6. Nutrition Services

• Screen children and pregnant or lactating women for acute malnutrition using national protocols.
• Identify and manage uncomplicated malnutrition cases according to IMAM guidelines.
• Refer complicated Severe Acute Malnutrition (SAM) cases to the appropriate stabilization center.

7. Mental Health and Psychosocial Support (MHPSS)

• Identify patients presenting with common mental health conditions.
• Provide basic psychosocial support and Psychological First Aid when appropriate.
• Assess patients for suicide risk whenever clinically indicated and initiate emergency referral procedures according to organizational protocols.
• Refer patients requiring specialized mental health care through established referral pathways.

8. Referral Services

• Identify patients requiring secondary or tertiary care.
• Stabilize emergency cases before referral whenever possible.
• Complete referral documentation accurately and ensure continuity of care.
• Coordinate with referral hospitals and follow up on referred patients where feasible.

9. Health Education and Health Promotion

• Provide individual and group health education sessions to patients and caregivers.
• Promote healthy lifestyles and disease prevention.
• Educate patients regarding medication adherence, follow-up appointments, nutrition, hygiene, immunization, breastfeeding, and maternal health.
• Participate in community health awareness activities in collaboration with Community Health Workers (CHWs).

10. Infection Prevention and Control (IPC)

• Adhere to standard infection prevention and control measures at all times.
• Practice appropriate hand hygiene and use personal protective equipment correctly.
• Support proper segregation and disposal of medical waste.
• Report infection prevention concerns to the Health Facility In-Charge.

11. Medical data collection and Reporting

• Maintain complete, accurate, and confidential patient medical records.
• Ensure consultation registers and patient files are completed correctly.
• Record diagnoses and treatments according to organizational standards.
• Contribute to weekly and monthly health reports as requested.
• Ensure timely reporting of epidemic-prone diseases and unusual health events.

12. Quality Improvement

• Participate in clinical audits, mortality reviews, and quality improvement initiatives.
• Support implementation of national treatment guidelines and organizational protocols.
• Participate in continuous medical education and professional development activities.
• Contribute to improving patient satisfaction and quality of health services.

13. Teamwork and Coordination

• Work closely with nurses, midwives, vaccinators, laboratory staff, nutrition personnel, pharmacists, PSS provider, and Community Health Workers.
• Participate actively in health facility meetings.
• Provide technical guidance and on-the-job mentoring to junior clinical staff when requested by the Health Facility In-Charge.
• Maintain effective communication with all members of the multidisciplinary team.

14. Emergency Preparedness and Response

• Participate in emergency medical response activities during disease outbreaks, natural disasters, and humanitarian emergencies.
• Support mobile health teams and outreach services when required.
• Participate in emergency preparedness planning and simulation exercises.

The HF Medical Doctor has to be flexible in performing his/her duties to ensure that beneficiaries are getting good quality services and perform other additional duties on request of the Health and Nutrition PM.
