Job Description

Main duties and responsibilities:

The Rehabilitation / Referral Officer is responsible for identifying vulnerable individuals, facilitating access to rehabilitation and essential services, strengthening referral pathways, and promoting disability inclusion, protection, and community resilience.

·        Identify and assess persons with disabilities and vulnerable individuals, conduct case assessments, and develop appropriate rehabilitation and referral plans based on their needs.

·        Facilitate referrals to rehabilitation centers, health facilities, psychosocial support, education, livelihood, protection, and other essential services while ensuring effective follow-up of referred cases.

·        Establish and maintain referral pathways and coordination with government institutions, Disabled Persons' Organizations (DPOs), NGOs, and relevant service providers to strengthen inclusive service delivery.

·        Maintain accurate beneficiary records, case files, referral databases, and documentation while ensuring confidentiality, data protection, and safeguarding principles.

·        Conduct community outreach and awareness sessions on disability rights, available services, rehabilitation support, and referral mechanisms.

·        Monitor referral activities, collect data, support monitoring and evaluation processes, and prepare timely weekly, monthly, quarterly, and donor reports.

·        Ensure compliance with organizational policies, safeguarding, PSEA, Child Safeguarding, Gender Equality, Code of Conduct, and perform other duties assigned by the Project Manager.

Job Requirements

Academic Qualifications

  • Bachelor's degree in Physiotherapy, Occupational Therapy, Social Work or a related field.
  • Minimum 2–3 years of experience coordinating assistive device provision, rehabilitation services, or disability inclusion referral pathways.
  • Experience working with NGOs, health facilities, rehabilitation centers, or Disabled Persons' Organizations (DPOs) is preferred.
  • Experience in case management, beneficiary referrals, and coordination with government and community stakeholders is an asset.

Language

  • Fluency in Pashto and Dari is required.
  • Working knowledge of English is an advantage.

Skills and Competencies

  • Knowledge of rehabilitation, disability inclusion, and referral systems.
  • Strong coordination, communication, and case management skills.
  • Good report writing and proficiency in Microsoft Office (Word, Excel, PowerPoint).
  • Commitment to safeguarding, inclusion, diversity, and accountability.
