About Action Against Hunger

Action Against Hunger (ACF) is a Non-Governmental Organization (NGO) that was created in France in1979. ACF has been active in Afghanistan in the medical, nutrition, food security, and water and sanitation fields since 1995. ACF improved access to health and nutrition services for the most vulnerable people, who live in remote areas without a localhealth system. In particular, the country team supported pregnant and breastfeeding women and children under five, who are atgreater risk of malnutrition. Nutrition programs rely on an integrated approach, addressing both primary and underlying causes, inorder to have a lasting impact on nutritional status. The country team worked at community level, in villages and at provincial level,strengthening the public health system. It also worked at national level to ensure reliable information about the nutritional situation reached the wider humanitarian community. Covid-19 prevention was integrated into all interventions and additional responseactivities were implemented, such as the disinfection of health facilities and public places, cash assistance, psychosocial support andthe distribution of hygiene kits to affected populations.

Job Description

Job Purpose

Provide clinical leadership in the TFU by consulting, diagnosing, treating, and monitoring patients according to National Protocols and international standards, while strengthening the capacity of Medical Doctors through on-the-job training, mentoring, and supportive supervision to ensure the effective application of medical and nutritional protocols in the management of patients in Stabilization Centers.

 

Responsibilities:

Objective 1: Ensure the provision of high quality medical and nutritional care in the TFU

A.       Duty Assignment:

§  The Pediatric Specialist will be assigned to the day shift from Sunday to Thursday and shall complete the required weekly working hours in accordance with the organization's Human Resources policies and procedures.

§  The Pediatric Specialist may be required to work night shifts and public holidays when necessary to support emergency response and ensure continuous quality care for TFU patients.

§  Conduct joint patient rounds with attending Medical Doctors to ensure accurate diagnosis, appropriate treatment, and adherence to national and internationally recognized clinical guidelines and protocols.

§  Provide continuous technical support, on-the-job training, mentoring, and capacity building to Medical Doctors, Nurses, and Nursing Assistants to strengthen clinical competencies and improve the quality of patient care in the TFU.

§  Lead and facilitate daily morning reports and technical case discussions on admitted patients.

§  Review patients’ medical records regularly, identify gaps in clinical and nursing care.

§  Carry out the daily rounds in the assigned ward – approximately 15-30 patients.

B.      Clinical responsibilities:

§  Clinical examination of patients daily in order to determine any change in their clinical status

§  Carry out any appropriate and available investigations that may adjust diagnosis

§  Prescribe medications and therapeutic foods according to patients’ diagnosis, age and weight, and be aware of possible drug interactions.

§  Ensure timely consultation and/or referral for complex medical cases which require higher medical support,

§  Re-evaluate critical patients regularly during the day and night

§  Carry out the admission visit, prioritize the emergency, take a comprehensive history, and clinical examination, medical and nutritional prescription

§  Perform night duties in TFU ward and assure quality and continuity of care 24/7

§  Support the treatment of complicated cases with medical doctor

 

Objective 2: Nutritional Responsibilities:

A) Admissions of eligible beneficiaries (children U5 with severe acute malnutrition with medical complications:

§  Decide which patients are at major risk and need special care in phase I.

§  Decide which patients should be admitted rapidly and examine after formal admission and the start of treatment.

§  Monitor, prevent and treat hypothermia.

§  Monitor, prevent and treat hypoglycemia.

§  Diagnose and treat dehydration were present (strictly using the protocol).

§  Follow up on the anthropometric measurement of the children (height, weight, MUAC, checking bilateral edema and W/H percentage) with a nurse or doctor.

§  Follow up on the registration of all the beneficiaries in the TFU (registration book ID number, medical and nutrition treatment sheets, follow-up card ID number).

§  Follow-up and implementation of the medical treatment for severe acute malnourished children in the IPD SAM.

 B) Follow up in the treatment phases in the TFU:

§  Calculation, preparation, distribution, and monitoring of therapeutic milk according to the standard protocols for the beneficiaries.

§  Follow up on the anthropometric measurements taken by the registrar according to the protocols, daily, each other day, or on weekly bases and record in the registration book, follow-up cards for each beneficiary.

§  Identified patient who needs to be transferred to another phase or OTP, patient with failure to respond to treatment and patients who need to be discharged.

§  Follow up of the gain weight and feedback to the caregivers.

C) AT discharge

§  Check if all the procedures are completed before discharge (anthropometric measurements, medical and nutritional treatment, systematic treatment, and health and nutrition education).

§  Record all the necessary information on the registration book and card for each beneficiary.

D) Health and Nutrition Education

§  Supervise health and nutrition education to the caregivers and beneficiaries on (breastfeeding, complementary feeding, hygiene, care of pregnant and lactating women, and prevention of illness).

 E) Reporting and statistics of the center

Keep a tally of all the beneficiaries admitted and discharged from the nutrition center (admission, cured, defaulters, death, transfer and referral) by age group and admission criterion used.

Job Requirements

SKILLS & EXPERIENCES REQUIRED FOR THE JOB

Education (if relevant) and experiences expected

-          Bachelor`s degree in Medicine (MD) and Specialization degree in Pediatrics from MoPH

-          At least one years’ experience as paediatrician post specialization

 

Technical skills

-          Previous experiences working in TFU is an asset

-          Patience, good relations with other people, good capacity to work in a team, precision, and under stressful situations.

-          Strong speaking and interpersonal skills to engage community members and build trust.  

-          Good team management and positive attitude

-          Fluent in Dari and Pashto (Written, spoken, Read)
