Job Description

Major Goal of the Position:         To supervise, support, and strengthen community health activities, including Community Health Workers (CHWs), public health programs, and facility–community collaboration, in order to improve community health outcomes and ensure effective implementation of BPHS community health components.

 

Key Result Area (KRA) 1:      Training, Support and Supervision of CHWs – the CHS will:

  • Assist in practical training during CHW training courses, including supervising the practical experience of the CHWs in the community during their training
  • Coordinate with the Project Management to ensure that on-the-job and monthly in-service training is provided to CHWs
  • Review and evaluate  the performance of the CHWs and identify need for further training
  • Assist the staff of the health facility in making plans for the community health programs in the facility and its catchment area.
  • Implement, supervise and evaluate the community health program activities in the catchment area of the health facility.
  • Identify and report immediately to the head of the health facility any problems that may interfere in achieving program objectives
  • Guide the CHWs in the development and implementation of their action plans.
  • Conduct monthly supervisory meetings with CHWs.
  • Ensure regular replacement of supplies in the CHW kits.
  • Conduct regular visits to the CHWs in their communities to assess and assist their work.
  • Encourage team work among CHWs, especially when they are working in the same catchment population
  • Provide regular reports on the CHWs to the head of facility

 

Key Result Area (KRA) 2:      Health Management Information System (HMIS) and Reporting – the CHS will:

  • Supervise the quality of the pictorial registers and Community Maps maintained by the CHWs and assist the CHWs where needed.
  • Supervise completion of the Monthly Activity Reports (MARs) by CHWs and the completion of the facility MAAR.
  • Consolidate the MAARs and assist in preparing consolidated monthly reports
  • Assist in maintaining graphs to monitor the facility health programs.
  • Assist in supervising any required community health survey
  • Use the reporting system and information received from Community Health Committees (Shura-e-Sehie) to monitor health conditions and submits findings through the established reporting channels

 

 

Key Result Area (KRA) 3:      Facility–Community collaboration – The CHS will :

  • Assist in the formation and strengthening of Community Health Committees (Shura-e-Sehie).
  • Provide orientation sessions to Shura members on BPHS and priority community health topics
  • Guide information sharing and implementation of community-based health activities.
  • Promote community support for CHWs and health facility initiatives.
  • Support communities in identifying, prioritizing, and addressing their health problems.
  • Provide regular feedback from the community to project management through the head of clinic

Job Requirements

Profile of CHS:

Required:

  • Degree in Public Health/ Nursing & Midwifery from recognized university.
  • At least 2 years’ experience in CHS position.
  • Respected and self-motivated Resident of the local community.
  • Strong interpersonal and communication skills.
  • Experience in community development, health care, or management will be an advantage.
  • Working knowledge of Dari or Pashto and Fluent in the local language if different.
  • Able and willing to travel extensively within the local area.
  • Conduct and appearance consistent with OpMercy values.
